1. Following the law of Thailand, Marriage Certificate is required.
2. In case of egg donation, must be following the condition as below:
   • Medical indication specified the reason why your egg cannot be used e.g. advance maternal age, recurrent IVF failure with unexplained cause, any cause effects to egg production.
   • Intended couple must find egg donors themselves. Service providers not allowed to get involved with egg trading, commissioning, compensation and advertisement of the egg donor.
   • Intended couple must send preliminary screening result of the egg donor to service providers for approval or bring her for the check-up before the treatment start.
   • An egg donor must hold marriage certificate or divorce certificate and she must get permission from her husband or partner with the written consent.
   • An egg donor age between 20-35 years old.
   • An egg donor must have the same nationality with the recipient (wife)
   • The recipient can receive donor eggs from only 1 donor at a treatment cycle.
   • The selected egg donor shall not donate her egg more than 3 times in her life.
3. In case of sperm donation, must be following the condition as below :
   • Married couple with medical indication to request for donor sperm treatment.
   • Intended couple must find sperm donors themselves. Service providers not allowed to get involved with sperm trading, commissioning, compensation and advertisement of the sperm donor.
   • Sperm donor must send preliminary screening for review: CBC, Hb Typing with Rule Out Alpha Thalassemia, ABO Blood Group, Rh Blood Group, DCIP, Anti HIV, VDRL, HBsAg, Anti HCV, Semen Analysis.
   • After service providers approved the preliminary screening results the sperm donor has to visit the clinic/hospital for sperm collection and freezing.
   • The collected sperm will be frozen for another 6 months. After 6 months, the sperm donor has to repeat infectious below and send the results for review: Anti HIV, VDRL, HBsAg, Anti HCV
   • If the results show that the sperm donor is totally free from infectious disease, the frozen sperm can be used for the treatment.
